Community and Habilitation Services

These programs provide various day programming and employment options for adults with intellectual and developmental disabilities, and support for individuals that are Nursing Home-eligible or have traumatic brain injuries, in addition to behavioral health services for adults with mental illness and/or alcohol and substance abuse. Programs also include services for children with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility, as well as their families. Eligibility varies by service.

Community and Habilitation Services

Adult Behavioral Health HCBS/CORE

Provides recovery and wellness services for adults 21 and over with mental health and/or substance use disorders that are enrolled in a Medicaid Managed Care Health and Recovery Plan (HARP).

Children and Family Treatment and Support Services

Over the past several years, New York State has been preparing to expand and transform children’s behavioral health services. These new mental health and substance use services, available with NYS Children’s Medicaid, give children/youth (under age 21) and their families the power to improve their health, well-being, and quality of life.

Children's Home and Community-Based Services (HCBS)

Provides services and supports certified by the New York State Office of Mental Health to families to prevent out of home placement for youth ages 0–21 years with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility.

Day Habilitation Services

Day Habilitation services are habilitative services that are provided through group activities out of the person’s home, and can assist individuals to acquire, retain or improve their self-help, socialization, and adaptive skills, such as communication, travel and adult interactions. Program options are available in Chemung and Steuben counties.

Provides recreational activities, information and referral services, and reimbursement for goods and services related to the child’s disability to families who have children with disabilities living at home.
