Recovery Coordination

Through this Recovery Coordination Agency (RCA), a Care Manager assists adults Medicaid Managed Care to gain peers and partners in the community. By collaborating with individuals in the community, the Care Manager provides ongoing assessment and referral for Adult Behavioral Health Home and Community-Based Services (HCBS). Pathways, Inc. is an approved provider of HCBS. These services are for adults aged 21 and up who are Health and Recovery Plan (HARP) eligible with behavioral health needs such as serious mental illness and/or substance use disorder. The adult may decide to be referred for coordination services to assist with HCBS and other needs, or the Care Manager can continue to monitor HCBS involvement.

  • Comprehensive assessment of behavioral health needs
  • Referral and linkage to appropriate home and community-based services
  • Ongoing assessment for Adult Behavioral Health HCBS

Population and Geographic Area Served

Recovery Coordination serves adults ages 21 and up who are HARP-eligible and have Medicaid Managed Care.

Access to Services

You can contact us any time to find out if you are eligible and to complete a self-referral. You can also be referred by other service providers and Managed Care Organizations, but your consent is needed to participate.
