Children's Care Management

Through Children’s Care Management, a Care Manager helps youth and caregivers get the care they need to improve the youth’s health. As part of the Health Home Care Management model, a group of health care and service providers work together to support children and youth who are enrolled in Medicaid and have qualifying health conditions. When you enroll your child in Children's Care Management, you are not enrolling in a place–you are gaining a Care Manager, the key point of contact in managing the services for your child.

Population and Geographic Area Served

The Children’s Care Management program serves children in the Southern Tier, Finger Lakes, and Western, New York regions from birth through age 20 who are enrolled in Medicaid and have two or more chronic health conditions (physical, developmental, mental health, or substance abuse diagnosis) or one single qualifying condition of HIV/AIDS, Serious Emotional Disturbance, or Complex Trauma.

Access to Services

You can contact us at any time to find out if your child/youth is eligible to enroll and to complete a self-referral. You can also be referred by Medicaid, Managed Care Plans, doctors, specialists, social service staff, counselors, or other service providers. If you are under age 21 and are pregnant, parenting, or married, or are over the age of 18, you can contact us at any time to complete a self-referral.

Children and Family Services

These services provide in-home, community, and foster care services and care coordination to youth with serious emotional disturbances, intellectual and developmental disabilities, and chronic medical conditions.

Children and Family Treatment and Support Services

These mental health and substance use services, available with NYS Children’s Medicaid, give children/youth (under age 21) and their families the power to improve their health, well-being, and quality of life. 

Children's Home and Community-Based Services (HCBS)

Children's HCBS is a strength-based program that serves youth ages 0–21 years with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility.

Therapeutic Foster Care

Become a Resource (foster) Parent and provide a local home to keep children and youth in their community.
