High Fidelity Wraparound is a national, evidence-based process that focuses on the goals of the family to address complex emotional and behavioral needs of the youth. The High Fidelity Wraparound Care Manager partners with the family and facilitates the intensive process with the collaborative team, including natural supports and community providers, while honoring family voice. The team builds the family-specific strengths to get needs met, learn new skills, and develop skills and supports to manage crisis.

Population and Geographic Area Served

High Fidelity Wraparound (HFW) serves youth ages 6 to 21 who reside in Chemung, Monroe, Ontario, Schuyler, Steuben, or Yates County, have a diagnosed Serious Emotional Disturbance or two mental health conditions, are involved in multiple child-serving systems to address behavioral health needs, and have recent or imminent risk of disruption of their living situation due to behavioral needs. The youth and family must be willing to actively participate in the intensive process throughout its four phases with the goal of the family learning to cooperatively manage stress together. All youth eligible for High Fidelity Wraparound must be Medicaid eligible.

Access to Services

Youth can be assessed for eligibility for High Fidelity Wraparound by their Children’s Care Manager through their regular CANS assessment. If eligible, your Care Manager can transfer your youth’s services to a local HFW Care Manager. If your youth does not have a Care Manager, complete a referral below!
