Lake Breeze Community Residence for Adolescents
Lake Breeze Community Residence for Adolescents, located in Canandaigua, NY serves youth who are experiencing emotional difficulties and have reached a level of stability and recovery in other inpatient and residential settings which enables them to live with others in a community-based setting, attend local schools, and participate in community, cultural, and social activities. This level of care works with families to empower and engage them in treatment so that the youth, when appropriate, may return home upon discharge.
The Lake Breeze CR program is licensed by the New York State Office of Mental Health.

Population and Geographic Area Served
The Lake Breeze Community Residence (CR) program serves children in the Finger Lakes, Southern Tier, and Rochester, New York regions between the ages of 13 and 18 years with serious and persistent symptoms of cognitive, affective, and personality disorders.
Access to Services
Referral packets for admission to the Lake Breeze CR may be obtained by contacting the Program Director. Completed referrals will be reviewed by a multidisciplinary Referral Review Committee to determine eligibility.
