High Fidelity Wraparound is a national, evidence-based process that focuses on the goals of the family to address complex emotional and behavioral needs of the youth. The High Fidelity Wraparound Care Manager partners with the family and facilitates the intensive process with the collaborative team, including natural supports and community providers, while honoring family voice. The team builds the family-specific strengths to get needs met, learn new skills, and develop skills and supports to manage crisis. This is a pilot program in Yates County made possible by NYS Systems of Care.

Population and Geographic Area Served

High Fidelity Wraparound (HFW) serves youth ages 5 to 20 who reside in Yates County, have a diagnosed Serious Emotional Disturbance, are involved in behavioral health services and/or multiple child-serving systems, and have emerging or imminent risk of disruption of living situation, childcare, or school placement due to behavioral needs. The youth and family must be willing to actively participate in the intensive process throughout its four phases. All youth eligible for High Fidelity Wraparound will be assessed for Medicaid eligibility.

Access to Services

You can contact us at any time to find out if your child/youth is eligible for assessment for High Fidelity Wraparound. Your child/youth can also be referred through Yates County school district staff or children’s Single Point of Accessibility, but cannot be assessed without your involvement.
