Through Adult Care Management, a Care Manager partners with adults to get the care they want to improve their physical, behavioral, and social health and supports. The Care Manager helps connect individuals with health care providers and service providers to reach personalized health goals. You must have Medicaid, qualifying health condition(s), and have an unmet need to be eligible for Adult Care Management, where you gain a Care Manager, the key point of contact in helping you manage your health needs. Pathways works in partnership with Circare, Huther Doyle, and Health Home of Upstate New York (HHUNY).

Population and Geographic Area Served

The Adult Care Management program serves adults age 18 and up in Chemung, Steuben, Schuyler, Tompkins, and Yates counties who are enrolled in Medicaid, Medicaid Managed Care, or HARP Plans and have two or more chronic health conditions (physical, mental health, or substance use diagnosis) or one single qualifying condition of HIV/AIDS, Serious Mental Illness, or Sickle Cell Disease. Pathways, Inc. is a designated Specialty Mental Health Care Management Agency that also serves Health Home Plus (HH+) qualified individuals who require the most intensive level of Health Home Services.

Access to Services

You can contact us any time to find out if you are eligible to enroll and to complete a self-referral. You can also be referred by Managed Care Plans, doctors, specialists, social services, counselors, or other service providers, though you cannot be enrolled without your consent.

Referral Options
Complete a referral form (based on your county residence) and submit by fax to (607) 937-3207.
Circare: Chemung, Tompkins county residents
Huther Doyle: Schuyler, Steuben, Yates county residents
