Family Support Services

Provides recreational activities, information and referral services, and reimbursement for goods and services related to the child’s disability to families who have children with disabilities living at home.

Nursing Home Transition and Diversion Service Coordination

Service Coordinators work in partnership with the individual to ensure that they receive the most appropriate supports based on their need and in the most integrated environment.

Traumatic Brain Injury and Nursing Home Transition and Diversion Support Services

Provides structured day programming, along with supports and services both in their own home, as well as in the community, to eligible individuals who are at least 18 years old.

Traumatic Brain Injury Service Coordination

Service Coordinators have been supporting individuals with TBI through the Department of Health’s TBI Waiver program since 1995, and work in partnership with the individual to secure supports and services that will allow them to live as independently and productively as possible.
