These services provide in-home, community, and foster care services and care coordination to youth with serious emotional disturbances, intellectual and developmental disabilities, and chronic medical conditions.

Adult Care Management

Through Adult Care Management, a Care Manager partners with adults to get the care they want to improve their health and supports. As part of the Health Home Care Management model through Huther Doyle and Health Home of Upstate New York (HHUNY), the Care Manager facilitates connections between health care and service providers to support individuals who are enrolled in Medicaid and have qualifying health conditions to remain in their homes and communities.

Children and Family Treatment and Support Services

Over the past several years, New York State has been preparing to expand and transform children’s behavioral health services. These new mental health and substance use services, available with NYS Children’s Medicaid, give children/youth (under age 21) and their families the power to improve their health, well-being, and quality of life.

Children’s Care Management

Serves children enrolled in Medicaid that have two or more chronic health conditions or one single qualifying condition of HIV/AIDS, Serious Emotional Disturbance, or Complex Trauma by providing a Care Manager who becomes the key point of contact in managing health care and services that the child needs to improve their health.

Children's Home and Community-Based Services (HCBS)

Provides services and supports certified by the New York State Office of Mental Health to families to prevent out of home placement for youth ages 0–21 years with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility.

Recovery Coordination

Through Recovery Coordination, a Recovery Coordinator assists adults in Steuben County with Fidelis Medicaid Managed Care to gain peers and partners in the community. By collaborating with individuals in the community, the Recovery Coordinator provides ongoing assessment and referral for Adult Behavioral Health Home and Community-Based Services (HCBS).

Therapeutic Foster Care

Serves youth in custody of the Department of Social Services (DSS) who have serious emotional or behavioral difficulties and/or developmental disabilities in need of a therapeutic out-of-home placement.

Additional Services

Enhanced Visitation

Provides quality visitation to assist parents in meeting their child’s developmental and attachment needs while strengthening the parent-child bond.
