Traumatic Brain Injury Service Coordination
According to “Brainline” a national multimedia project offering information and resources on Traumatic Brain Injury (TBI), there are 2.8 million individuals diagnosed with TBI annually in the United States alone. Ninety percent (2.5 million) are treated and released. Most are diagnosed with mild TBI best known as concussion.
Pathways, Inc. TBI Service Coordinators have been supporting individuals with TBI as one Service Coordination agency through the Department of Health’s TBI Waiver program since 1995. The TBI Service Coordinator works in partnership with the individual to secure supports and services that will allow them to live as independently and productively as possible.
Services Provided
- Meets with you in your home
- Provides education on available services
- Promotes choice
- Secures needed assessments to maintain eligibility
- Assists in obtaining and maintaining benefits and entitlements (Medicaid, Medicare, Food Stamps)
- Links to doctors, specialists, day services, and transportation
Population and Geographic Area Served
The Traumatic Brain Injury (TBI) Service Coordination program supports individuals in Broome, Chemung, Schuyler, Steuben, Tioga, and Tompkins counties under the guidance of the Department of Health’s TBI Waiver. Individuals must be between the ages of 18–64, Medicaid-eligible, and have a documented TBI.

The TBI Service Coordinators are trained to have a clear understanding of the brain. They are knowledgeable of the service delivery system and resources available in the community.
