Nursing Home Alternative (Nursing Home Transition & Diversion)
Pathways, Inc. provides supports and services to people who would typically require placement in a nursing home facility to maintain their health and safety. The Nursing Home Transition and Diversion (NHTD) Waiver allows individuals to receive oversight and supervision in their own home instead of a nursing home. Individuals wishing to leave a nursing home setting and return to their home community or to prevent placement in a nursing home who meet the eligibility requirements may participate in this program.
Eligibility: Individuals residing in Broome, Chemung, Schuyler, Steuben, Tioga, or Tompkins Counties who are age 65 years or older or between 18 and 64 years with a diagnosed disability (including but not limited to Alzheimer’s dementia, MS, spinal cord injury) are eligible to participate in the NHTD Waiver program. Home and Community Support Services are provided through Pathways, Inc. Licensed Home Care Services (LHCSA). A Registered Nurse will provide oversight to all orders from the participant’s medical practitioner recommending oversight, supervision, personal care services, mobility, meal preparation, and or light housekeeping tasks. Services are paid for through Medicaid or can be privately paid.

• Structured Day Program
