TBI/NHTD/Home Care Support Ser

Status: Part time

Nature Of Work:
• This position will primarily be responsible for coordinating the community-based health care services, home visits, and assessments of our specified consumer group. The RN will develop, implement, coordinate, and evaluate health care plans in an outpatient environment. It is also expected that the RN will collaborate with other health care providers and community service agencies to maintain updated health care plans and required documentation. The RN will clinically supervise all staff that performs HCSS/PCA duties. The RN qualifies to be an Independent Living Skills Training and Development staff. The RN position will directly work with the ILSTD staff in the TBI/NHTD Services. The RN will collaborate with the ILSTD and/or ILSTD Supervisor regarding ongoing and yearly assessments. The RN could be asked to provide ILSTD services to complete initial/annual assessments and may provide services in place of an ILSTD that is off or on vacation. In addition, the RN may do health counseling with consumers and/or their families as requested through the Waiver Services. He/She must have excellent communication skills and capable of fostering relationships that value and support others. They must also be a self starter and be capable of functioning with minimal supervision.

Minimum Qualifications:
• Current RN license with the New York State Department of Education.
• Have at least two years satisfactory recent home health care experience.
• A valid driver’s license passing insurability guidelines.
